A page of questions is interleaved between pages 4 and 5 of this document. This unnumbered page is a legal-size sheet folded up at the bottom to match the other pages in size. The fold is bound into the document book, making it difficult to read questions 10-13, as well as the third, fourth and fifth lines of question 9. This page bears a long note, handwritten in ink, at the top of the page, as follows: "To: Lt. Col. Van Cleve / Interrogation Office - Ashcan. / Please put the following questions up to Field Marshall's [sic] Keitel and Kesselring orally and send results of individual interrogations to Gen. Betts, personal, SHAEF forward. Following [crossed out] Request action by Monday, 21 May. Following not to be divulged to Germans: Information sought has been requested on short notice by US War Dept. / Basic Decisions of German High Command 1940-45." Both the sheet and the document proper are typewritten carbon copies of excellent quality on thin, slightly browning paper. Each page is marked "SECRET."
